GLP-1 weight-loss injections and hair loss: why Mounjaro and Wegovy users shed and what helps
Ozempic, Wegovy or Mounjaro and your hair: what 2026 studies show about GLP-1 weight-loss injections and shedding, why it happens, how long it lasts and what to do in Malaysia.
Weight-loss injections have arrived in Malaysian clinics in a big way. Semaglutide, sold as Wegovy for weight management, became available here in January 2026, and tirzepatide, sold as Mounjaro, was registered by Malaysia's drug authority in 2024 1 2. As more people start these medicines, a new question keeps coming up in clinics and online groups: "My weight is finally coming down, so why is my hair falling out?"
This guide explains what the research now says about GLP-1 medicines and hair loss, why the shedding happens, how to tell harmless shedding from something that needs a doctor, and what a sensible plan looks like for a Malaysian reader. It is general information, not personal medical advice, and it is not a reason to stop a medicine your doctor prescribed. For background, see hair loss after weight loss and diabetes and hair loss.
The short answer
- Hair loss is a recognised, fairly uncommon effect of GLP-1 medicines. A 2025 meta-analysis of 34 randomised trials found alopecia was reported about 3.4 times more often on GLP-1 medicines than on placebo, with the signal driven mainly by tirzepatide trials 3. A 2026 meta-analysis put the overall rate of hair loss at about 3.9% of users 4.
- Most of it looks like telogen effluvium, a temporary, diffuse shedding that starts a few months after a shock to the body, such as fast weight loss, a sharp drop in food intake or illness 5 6.
- It usually settles. Telogen effluvium tends to ease within 6 to 9 months once the trigger is over, although recovery can be incomplete for some people 6.
- Large record studies suggest it may not be only about weight loss. Some found higher rates of pattern hair loss too, and one found the extra risk even where weight change was modest 7 8. These studies show an association, not proof of cause.
- Do not stop or change the dose on your own. Talk to the prescribing doctor, check protein and iron, and get a scalp assessment if shedding is heavy, patchy or lasts beyond six months.
What GLP-1 medicines are
GLP-1 (glucagon-like peptide-1) is a hormone the gut releases after a meal. It tells the brain you are full, slows the emptying of the stomach and helps the body release insulin when blood sugar rises. GLP-1 receptor agonists copy this hormone. Semaglutide and liraglutide act on the GLP-1 receptor; tirzepatide acts on both the GLP-1 receptor and a second gut hormone receptor called GIP.
These medicines were first used for type 2 diabetes, and higher doses are now used for weight management. In Malaysia, Wegovy is approved as an add-on to a reduced-calorie diet and more physical activity for adults with obesity, or with overweight plus at least one weight-related condition, and it is a weekly prescription injection 1. Mounjaro pens and vials were registered by the Drug Control Authority in 2024 2.
The key point for hair is simple: these medicines make people eat much less, often quickly. That change, more than the injection itself, is the most likely link to shedding.
How hair grows, and why sudden change makes it fall
Each hair follicle runs through a cycle. The growth phase (anagen) lasts years on the scalp, and at any time most hairs are in it. A short transition phase (catagen) follows, then a resting phase (telogen) of about three months, after which the old hair is pushed out by a new one. Losing roughly 50 to 100 hairs a day is normal 9.
Hair is one of the fastest-dividing tissues in the body, and it is also one the body treats as optional. When the body senses a shock, such as rapid weight loss, a big fall in calories or protein, surgery, a high fever or severe stress, many growing follicles can switch into the resting phase at the same time. Nothing seems to happen at first. Then, about two to three months later, those resting hairs fall together. This is telogen effluvium 6.
That delay is why people often blame the wrong thing. Someone who started an injection in March may notice heavy shedding in May or June, just as the weight loss is going well. The follicles are not dead; they have paused. Once the body settles at a steady intake and weight, they usually return to the growth phase, and new hairs start the slow journey back to their old length.
Other ways GLP-1 treatment may affect hair
- Too little protein. Hair is made mostly of keratin, a protein. A very small appetite can mean too little protein for months.
- Low iron and other nutrients. Eating less overall can lower iron, zinc and other nutrients. Iron deficiency is a well-known, treatable contributor to shedding, especially in women 10.
- Unmasking pattern hair loss. In people with a genetic tendency, a period of shedding can make thinning at the crown or parting obvious for the first time. Some cohort studies found higher rates of androgenetic alopecia in GLP-1 users, possibly linked to metabolic and hormonal changes 8 7.
- A possible direct effect. Researchers have raised the idea that the medicines may act on follicles directly, but this is a hypothesis, not an established fact 8.
What the research shows
The evidence has grown fast. Here are the main studies, in plain terms.
Randomised trials, pooled (2025). A meta-analysis of 34 placebo-controlled trials with 84,243 participants found alopecia in 6.0 per 1,000 patient-years on GLP-1 medicines, against 0.8 on placebo, a risk ratio of 3.40. Tirzepatide trials drove most of the difference. Skin effects were not a planned endpoint in these trials, so hair loss may have been under-reported 3.
A second meta-analysis (2026). Pooling nine interventional studies with 4,114 GLP-1 users, researchers found a risk ratio for hair loss of about 3.25 against placebo, and about 3.6 when they looked only at trials in people with overweight or obesity. About 3.9% of users reported hair loss 4.
Large health-record studies (2026). Several studies used TriNetX, a database of anonymised medical records from many countries:
- One compared over 1.1 million matched patients on semaglutide or tirzepatide with patients on metformin. It found higher risks of telogen effluvium, pattern hair loss, alopecia areata and other non-scarring hair loss, while weight change over time was modest, which suggests rapid weight loss may not be the whole story 7.
- Another, with about 548,000 patients in each group, found that by 12 months GLP-1 users had higher odds of telogen effluvium and pattern hair loss, but not of alopecia areata 8.
- A third found the strongest link with telogen effluvium, and noticed that people who developed hair loss had lower follow-up BMI than those who did not, which supports the idea that the stress of weight loss plays a part 5.
How to read this. The studies agree that hair loss is more common in GLP-1 users. They disagree on details, such as alopecia areata, and record studies rely on diagnosis codes, cannot measure severity and cannot prove cause 7. For an individual, the practical message is the same: shedding is possible, it is usually temporary, and it is worth tracking.
Types of hair loss you might notice
| Pattern | What it looks like | Likely link to GLP-1 treatment | What to do |
|---|---|---|---|
| Telogen effluvium | More hairs on the pillow, in the shower and on the brush; thinning all over, with no bald patches; starts 2 to 3 months after a trigger | The most common type reported 5 | Check diet and blood tests; track for 3 to 6 months 6 |
| Pattern hair loss (androgenetic alopecia) | Gradual thinning at the crown, temples or a widening parting | Higher rates in some studies; shedding can unmask it 8 | See a doctor; it does not fade on its own |
| Alopecia areata | Smooth, round bald patches | Mixed findings across studies 7 8 | See a dermatologist promptly |
| Breakage | Short broken hairs, rough ends, no root bulb | Not a medicine effect; often heat, bleach or dryness | Gentler styling and care |

What to expect, step by step
- Months 0 to 2 after starting. Appetite drops and weight begins to fall. Hair usually looks normal.
- Months 2 to 4. If telogen effluvium is going to happen, shedding usually starts now. It can be alarming: clumps in the shower drain, a thinner ponytail, more scalp showing under bright light.
- Months 4 to 6. Shedding often peaks and then slows, especially once weight loss slows and eating becomes more regular.
- Months 6 to 12. New short hairs often appear along the hairline and parting. Volume returns gradually, because scalp hair grows only about a centimetre a month.
- Beyond 6 months of heavy shedding. If shedding is not easing, or thinning is in a pattern, it may be chronic telogen effluvium or pattern hair loss, and it needs a proper assessment. See chronic telogen effluvium.
Pros, cons and risks of the main options
Waiting and tracking. Works for most mild telogen effluvium. It costs nothing, but it can be stressful, and it can miss pattern hair loss that will not fade on its own.
Fixing nutrition. Enough protein and correcting a proven deficiency address the most common contributors. The risk is over-supplementing: high doses of vitamins such as A, or iron taken without a confirmed low level, can do harm. Test first. See blood tests for hair loss and iron supplements for hair loss.
Adjusting the weight-loss plan with the doctor. Slower dose increases or a slower rate of loss may reduce the shock to the body. Only the prescribing doctor should make this call, weighing hair against the health benefits of weight loss.
Topical minoxidil. It can help pattern hair loss and is sometimes used to support recovery from shedding. It can cause scalp irritation and an early temporary shed. See minoxidil side effects.
In-clinic scalp care and treatments. A scalp assessment can confirm what kind of hair loss it is and track density. Non-surgical treatments vary in evidence; ask what each one is meant to do and how progress will be measured.
Hair transplant. Not for shedding. Transplants are for stable pattern hair loss, and operating during active telogen effluvium makes it hard to judge the donor area and the result.
Typical timelines
- Shedding onset: about 2 to 3 months after weight loss starts 6.
- Shedding duration: usually eases within 6 to 9 months once the trigger settles 6.
- Visible recovery: new growth is often seen within months, but full length and volume take longer because hair grows slowly.
- Pattern hair loss: does not resolve without treatment; effects of treatment are judged over 6 to 12 months.
The Malaysian context
The medicines are here, and so are the shortcuts. With Wegovy and Mounjaro registered and widely discussed, injections are also sold through unofficial channels. Only use products with a valid registration, prescribed after a proper check; you can verify registration on NPRA's website 11.
Malaysian diets and appetite. Many popular meals here are rice- or noodle-heavy, with a modest portion of protein. When appetite shrinks, it is easy to finish the rice and leave the chicken, fish, egg or tofu. On a GLP-1 medicine, putting protein first on the plate is a simple habit that helps hair and muscle.
Iron and women. Heavy periods, pregnancy and low intake of red meat make low iron common in women. Ask the doctor about a ferritin test if shedding starts; see ferritin and women's hair loss.
Heat and humidity. A hot, humid climate does not cause telogen effluvium, but an oily, itchy scalp can make shedding feel worse and can hide a problem such as seborrhoeic dermatitis. Wash often enough to keep the scalp comfortable.
Fasting months. During Ramadan, a small appetite plus fasting hours can mean very little food. Plan sahur and iftar around protein, and discuss the injection schedule with the doctor beforehand.
Cost. This article does not list prices; costs for medicines, tests and scalp treatments vary by clinic and pharmacy.
Where to go. Start with the doctor who prescribed the injection. For hair concerns, a GP can order blood tests, and a dermatologist registered with the Malaysian Medical Council can confirm the diagnosis; the MMC register lets you check a doctor's registration 12. Hair clinics in the Klang Valley, Penang and Sarawak can offer a scalp assessment and density tracking.
Common myths
- "The injection is poisoning my hair follicles." The most common pattern is telogen effluvium, a temporary pause triggered by stress on the body 5 6.
- "If my hair falls, I must stop the medicine." Not without advice. Stopping can undo the health benefits, and shedding that has already been triggered will not stop at once.
- "Everyone on these drugs loses hair." Most users do not. Pooled trial data put hair loss at a few per cent 4.
- "Biotin will fix it." Biotin helps only if you are deficient, which is rare, and high doses can distort some blood tests. See biotin and hair loss.
- "Shedding means I am going bald." Diffuse shedding usually settles. Thinning in a pattern is different and worth checking.
- "It is only about the weight loss." Weight loss is a big part of it, but some large studies suggest it may not be the whole explanation 7.
A practical checklist
- Note the dates. When you started the injection, each dose change, and when shedding began.
- Take baseline photos. Parting, crown and hairline, same light and angle, every month.
- Put protein first at every meal, even when you are not hungry. See protein and hair.
- Ask for blood tests if shedding is heavy: a full blood count, ferritin, thyroid function, and others the doctor suggests 10.
- Do not self-prescribe supplements in high doses. Correct proven deficiencies only.
- Tell the prescribing doctor about shedding before changing anything.
- Be gentle with your hair. Avoid tight styles, harsh bleaching and very hot tools while it recovers.
- Get a scalp assessment if you see patches, a pattern, scalp redness or pain, or if shedding lasts beyond six months.
- Buy injections only through registered, prescribed channels 11.
The bottom line
Hair loss on GLP-1 medicines such as semaglutide and tirzepatide is real but uncommon, and most of it is telogen effluvium, a temporary shed that starts a few months after rapid weight loss and usually settles within six to nine months. The newest large studies suggest the medicines may also be linked with pattern hair loss, so thinning that follows a pattern or does not settle deserves a proper look. Keep the medicine decision with your doctor, eat enough protein, check iron, track your hair with photos, and get a scalp assessment if things do not improve.
Frequently asked questions
Do Ozempic, Wegovy or Mounjaro cause hair loss?
Hair loss is a recognised but fairly uncommon effect. Pooled trial data put it at roughly 4% of users, more often with tirzepatide (Mounjaro) than with placebo.
Why does hair fall out on GLP-1 injections?
Mostly because rapid weight loss and eating much less push many follicles into the resting phase at once, causing telogen effluvium two to three months later. Low protein or iron can add to it.
Is GLP-1 hair loss temporary?
Telogen effluvium usually eases within 6 to 9 months once weight and eating settle. Thinning in a pattern, or shedding beyond six months, needs a doctor's assessment.
Should I stop the injection if my hair is falling?
Not on your own. Speak to the prescribing doctor, who can weigh the health benefits and may adjust the dose increase or rate of weight loss.
Which blood tests help?
A doctor may check a full blood count, ferritin (iron stores) and thyroid function, plus other tests depending on your history.
Will supplements like biotin help?
Only if you are deficient. Correct proven deficiencies and eat enough protein; high-dose supplements without testing can cause harm or distort test results.
Is Mounjaro or Wegovy registered in Malaysia?
Wegovy became available in Malaysia in January 2026 and Mounjaro was registered in 2024. Use only registered products prescribed by a doctor, and check registration on NPRA's website.
Can I do a hair transplant if GLP-1 shedding does not stop?
Transplants are for stable pattern hair loss, not active shedding. Get a proper diagnosis first and wait until shedding has settled.
